This was the third MAIB investigation into a fishing vessel skipper falling asleep alone on watch (after Sustain, 2023, and Crystal Stream, 2025). Why did MAIB make no new recommendations?
A. Because no crew were injured in this particular accident
B. Because the safety issues are already well understood from previous investigations and existing guidance already covers them
C. Because the vessel was a total loss, so recommendations were considered moot
