Federal Indiana: The Space Nobody Classified as Dangerous

What Happened
On 19 February 2025, the bulk carrier Federal Indiana was underway in the Mediterranean Sea, laden with scrap metal across six cargo holds — a cargo the Company had itself assessed as oxygen-depleting and capable of emitting toxic gas. Each hold was accessed by two “booby hatches” on the main deck. The forward booby hatch for Cargo Hold No.1 sat inside a small space under the forecastle deck — roughly 3 by 1.5 metres, reached through a weathertight door, and used to store spare pallets and cleaning chemicals.
That space was naturally ventilated. Under the IMO resolution in force at the time, that single fact meant it met neither the definition of an “enclosed space” nor an “adjacent connected space” — so it never appeared on the ship’s enclosed-space list, and no gas test or entry permit applied to it.
That morning, the Bosun and a Deck Trainee Seafarer (DTS) opened the booby hatch covers on Holds 1 and 2 so the Chief Officer could inspect the toggle pins and packing later that day. Around 0830, after helping open the hatch, the DTS was told to join another rating nearby once he finished. He never arrived. When the Bosun passed the access space again shortly before 0904, he found the DTS collapsed inside.
A Rescue That Nearly Became a Second Casualty
The alarm was raised immediately. The first two crewmembers to reach the space both struggled: a fitter felt his breath “taken away” and retreated for fresh air; an Able Seafarer Deck (ASD) who entered to help the DTS became dizzy, short of breath, and lost coordination — he only got himself back out before losing consciousness, and needed 20 minutes of medical oxygen to recover. It was the Bosun and Chief Engineer, donning self-contained breathing apparatus, who finally extracted the DTS.
CPR continued from 0904 until the DTS was airlifted at 1205, after the Master diverted toward Skikda, Algeria, for a helicopter medevac. He was declared deceased on arrival ashore at 1300. A later gas survey, taken on 25 February with the hatches closed, found high concentrations of carbon monoxide across five of the six holds — and elevated hydrogen cyanide, unusual for a scrap metal cargo and still not fully explained by the investigation.
The Regulatory Gap Behind It
This is the finding that makes Federal Indiana matter beyond one ship. IMO Resolution A.1050(27), the rule in force on the day, defined an “adjacent connected space” as one that was normally unventilated. A space with natural ventilation — like the Hold No.1 access space — simply fell outside that definition, no matter how directly it opened onto a hold that could kill you. The Marshall Islands investigation found the same reasoning applied fleet-wide, on other ships in the Company’s fleet with similar layouts around crane foundations and booby hatches.
That loophole has since been closed. IMO Resolution MSC.581(110), adopted 27 June 2025 and in force since 3 December 2025, replaces A.1050(27) and introduces a “connected space” category that must be treated as hazardous until testing proves otherwise — ventilation status no longer exempts it. The investigation also found the IMSBC Code gives no guidance at all on the gas-forming hazards of scrap metal, and has recommended the Marshall Islands propose an amendment to close that gap too.
Recommended Actions
- Treat any space with an opening into a cargo hold, tank, or void as a connected space regardless of its own ventilation — test before entry, every time (per IMO Resolution MSC.581(110), now in force).
- Audit your ship’s enclosed/connected space list against every space with a physical opening into a hold — not just the holds themselves. Federal Indiana’s access space was never on the list.
- Never enter a space to rescue a collapsed crewmate without breathing apparatus — a second untrained entry risks a second casualty, as very nearly happened here.
- Practise enclosed-space rescue drills as if they are real emergencies, with SCBA donning and stretcher handling under time pressure, not as a paperwork exercise.
- Flag any conflict between a shipper’s Cargo Information Form and the IMSBC Code (e.g. loading scrap metal in precipitation) rather than accepting it on the basis of “customary practice.”
Human Element Analysis
Situational Awareness
The access space was opened and occupied as part of a routine, low-drama maintenance task. Once its only barrier to the hold — the booby hatch cover — was removed, nobody re-assessed the space itself as newly hazardous.
Normalisation
The space had been entered “one or two days before, without incident,” and was in everyday use for storage. Repeated safe entries built confidence in a space whose actual risk hadn’t changed at all.
Complacency
“Naturally ventilated” was treated — by this ship’s SMS and by sister ships in the same fleet — as if it were a safety feature, when it was really just a definitional loophole that happened to exempt the space from testing.
