The Situation
You are the Chief Officer aboard a laden cape-size bulk carrier in the East China Sea, a day out from arrival. This scenario takes you through the whole task from start to finish: the plan, the risk assessment review, the permit to work, the job site review and the job itself. Your earlier choices carry forward.
The Lesson
The controls are a chain: plan the scope, test the risk assessment against the real equipment, set the permit trigger, brief and check the site, and call Stop Work in the first second. Anchor to the ship, never the equipment.
Human Element
A sheave that couldn't easily be inspected, clean paperwork that hid the failure, an unassigned job that grew over the side without a permit, a harness clipped to the thing that fell, and colleagues who saw it and didn't call Stop Work. Five linked stages, each a chance to catch it.
