The Situation
You're the second officer aboard a general cargo vessel discharging at the Port of Seville. You had drinks ashore before coming back for cargo watch, and you can feel the effects. The watch still needs covering, and the chief officer is expecting you on deck.
The Lesson
Exclusion zones during cargo crane operations must be physically enforced. A crew member who knows where the danger zone is cannot be relied on to stay out of it, especially if their judgement is impaired.
Human Element
Alcohol impaired the officer's judgement of the danger zone around the gantry crane, and the chief officer supervising the operation had no way of knowing where he was — a coordination gap made worse by an initial failure to report the accident at all.
