The Situation
You're the skipper of a fishing vessel moored at Kilkeel Harbour for the night. As you secure the boat before turning in, you notice the mess deck hatch is open for ventilation, as it usually is — but since a modification to the boat's layout, the access route to the toilet now passes directly over it.
The Lesson
Layout modifications that create new hazards in crew movement areas must be risk-assessed and fitted with physical safeguards. Familiarity with a hazard is not the same as managing it.
Human Element
The open hatch had become an accepted feature of the vessel through repeated, incident-free familiarity, and the post-build layout change that routed the access path directly over it was never formally risk-assessed after the modification was made.
