The Chief Engineer requests hot work (welding) in the engine room fuel oil overflow tank area for 2 days at sea. As Chief Officer, what is your role in authorising this?
A. HOT WORK PERMIT — Chief Officer authorisation role: (1) ASSESS LOCATION: the fuel oil overflow tank area is a HIGH RISK hot work location — potential residual fuel vapour; (2) PRE-CONDITIONS before issuing permit: (a) Gas-free certificate: area must be tested for flammable vapours by a competent person (gas detector); LEL <1% for hot work in enclosed spaces — otherwise work cannot proceed; (b) Area cleared of combustibles — drip trays removed, fuel residue cleaned; (c) Fire watch designated — a dedicated person watching for sparks, overheating, fire during and after work; (d) Fire extinguisher type C (CO2 or dry powder) at the work site; (e) Hose ready at standby; (f) Adequate ventilation; (g) Hot work permit signed by Chief Officer AND master; (2) DURATION: the permit is typically issued for a maximum of 8-12 hours (one working day); must be renewed each day with a new atmosphere test; (3) POST-WORK: the FIRE WATCH continues for a minimum of 30-60 minutes after work stops — hot metal can ignite materials long after the welder has left; (4) PERMIT CONTENT: identifies the location, type of work, duration, fire watcher's name, safety measures, atmosphere test result, signatures.
B. Hot work authorisation is entirely the Chief Engineer's responsibility. As Chief Officer, counter-sign the permit when the Chief Engineer presents it to you.
C. Welding in a fuel tank area only requires a gas-free certificate. Once gas-free, work can proceed without a permit or dedicated fire watch.
D. Hot work permits are only required in Class A division spaces (cargo hold fire boundaries). Engine room hot work is covered by normal engine room operating procedures.

A. ENGINE ROOM FIRE — Chief Officer management: (1) TAKE COMMAND of the fire response — as Chief Officer (typically Fire Commander in the SMS muster plan), you assume command of the fire-fighting team while the master commands the vessel from the bridge; (2) IMMEDIATE ASSESSMENT: confirm with engineering watch whether the fire is in the engine room proper — is there smoke/heat at the engine room access? Do NOT open the access door if fire is suspected — this admits oxygen; (3) STOP VENTILATION: order the bridge to shut the engine room ventilation remotely — starve the fire of air; close fire dampers; (4) MUSTER: ensure fire party is mustered with BA (breathing apparatus) sets donned and tested; boundary cooling team ready with hoses at engine room boundaries; (5) CO2/FIXED SYSTEM: if the fire cannot be controlled quickly, prepare to activate the CO2 fixed firefighting system — this requires: (a) engineer confirms all personnel are evacuated from the engine room; (b) master's authority to activate; (c) PA announcement: "All persons stand clear of Engine Room, CO2 release imminent"; (d) activate from the CO2 cabinet (outside the engine room); (6) BOUNDARY COOLING: maintain water spray on all bulkheads adjacent to the engine room — prevent fire spread to accommodation and other spaces; (7) COMMUNICATIONS: regular sitrep to master every 5 minutes; (8) ABANDON SHIP READINESS: if CO2 fails or fire spreads — initiate abandon ship preparation.
B. Immediately open the engine room access door to assess the fire. Visual inspection is necessary before any other action can be taken.
C. The Chief Engineer takes command of all engine room fires. The Chief Officer's role is only to ensure the deck crew are mustered and ready if abandon ship is needed.
D. Activate the CO2 system immediately without checking for personnel inside. Speed of response is more important than personnel headcount in an engine room fire.

A. ACCOMMODATION FIRE management: (1) CONFIRM MUSTER: first priority — account for all crew; check the muster roll; if any person is unaccounted for, treat as missing until confirmed evacuated or rescued (do NOT assume they evacuated); (2) ISOLATE THE FIRE: shut all accommodation ventilation immediately; close all cabin doors in the affected section — closed doors significantly delay fire spread (a standard cabin door can hold fire for 30-60 minutes); (3) ATTACK THE FIRE: send BA team in pairs (minimum two persons) to the cabin of origin; carry charged hose lines; do not enter without BA — smoke contains CO, HCN, and other toxic gases regardless of whether visible flames are present; (4) SEARCH AND RESCUE: if any person is unaccounted for and may be in the fire area — BA team conducts a search; confirm all adjacent cabins are empty; (5) CLOSE VENTILATION DUCTS: identify and close the HVAC dampers for C-deck; (6) ELECTRICAL ISOLATION: consider isolating electrical supplies to the affected section — fire may be electrical in origin and live wiring in a flooded space is lethal; (7) SITREP to MASTER every 5 minutes; (8) PA SYSTEM: broadcast calm instructions to crew — direct them to muster stations if not already there; (9) BOUNDARY COOLING: above and below C-deck — prevent vertical spread through the deck structure.
B. The crew can fight the fire themselves using portable extinguishers. The Chief Officer only activates the BA team if portable extinguishers fail after 10 minutes.
C. Open all accommodation doors and windows to ventilate the smoke away from the fire area. This clears the escape routes for evacuating crew.
D. Electrical isolation is not a priority — fire in accommodation is almost always caused by smoking materials, not electrical faults.
