Maritime Questions › DP — Risk Assessment
When preparing a task-specific risk assessment for a DP operation (e.g. a subsea lift), what DP-specific inputs must the risk assessment draw on, beyond generic task hazards?
A. Only the crew's certification and rest hours
B. The vessel's WCF, current ASOG limits, footprint/consequence analysis for the specific location, position reference availability, and proximity to fixed/floating structures or other vessels
C. Only the weather forecast for the next 24 hours
D. DP-specific factors are not part of a task risk assessment — they are covered separately in the FMEA

A. A HAZID (Hazard Identification) is a broad, early-stage exercise to identify what could go wrong across the whole operation; a HAZOP (Hazard and Operability study) is a more structured, systematic review of a specific system or procedure using guide words to probe deviations from intended operation
B. A HAZID is only used for diving operations; a HAZOP is only used for drilling operations
C. A HAZOP is conducted by the DPO alone; a HAZID requires the whole bridge team
D. They are interchangeable terms for the same process required by different classification societies

A. The permit only needs Chief Engineer sign-off; the bridge does not need to be informed if the work is in the engine room
B. DP operations must always be suspended before any permit-to-work is issued, regardless of what the work involves
C. Whether the work could affect any system relied on for the current WCF or active redundancy — the permit must be reviewed against the DP system status before issue, and the DPO/Master must be consulted and aware of any DP-critical system being isolated or worked on
D. Hot work permits are entirely independent of DP status and only concern fire safety
