What It Is
Every year, the NTSB's Office of Marine Safety publishes a digest summarising the circumstances, probable cause, and lessons learned from every marine casualty investigation it completed the previous year. The 2025 edition covers 49 investigations — capsizings, collisions, contacts, fires, floodings, groundings, and machinery damage — including two of the highest-profile marine casualties in recent memory: the containership Dali's collision with Baltimore's Francis Scott Key Bridge, and the implosion of the submersible Titan. Those two cases have already had the full case-study treatment across the industry. What's less visible is the other 47.
Here are six cases from this year's digest that don't carry a famous name, but carry a lesson worth five minutes of your attention.
Baylor J. Tregre — Capsizing of a Towing Vessel in a Squall
Towing the barge MARMAC 27, loaded with a production platform and helideck, off Galveston, Texas, Baylor J. Tregre was overtaken by a sudden storm. Wind on the barge and its cargo halted the tug's forward motion and dragged it sideways; the tow heeled 45° to port before the crew abandoned. NTSB's report flags deck-edge immersion — when the main deck edge goes underwater — as a critical warning sign: it typically coincides with a vessel's maximum righting arm, after which further heeling risks a full capsize.
Lovebug — A Partially Open Garage Door Sank a Yacht in Four Minutes
An external "garage door" at the stern of the yacht Lovebug was found partially open after the vessel began listing in Chesapeake Bay. The watertight door between the garage and the engine room compartment was also open. Once water reached the lower edge of that open watertight door, it could flow freely between spaces — and once it did, the yacht listed to 45–50° and everyone aboard was in the water within four minutes of the list beginning.
Papa Rod / Appaloosa — A Collision, and a Positive Drug Test
The fishing vessel Papa Rod struck the anchored bulk carrier Appaloosa in the Gulf of America while running with no set watch schedule. A deckhand's post-accident urine test came back positive for methamphetamine, amphetamine, and THC metabolite — at levels the NTSB noted "generally increase the likelihood" of recent abuse, though toxicology alone couldn't establish impairment at the time of the collision. The finding doesn't prove cause on its own, but it's a reminder that watch discipline and substance policy aren't independent issues.
William B Klunk — An 18-Hour Day and a Phone Call
The pilot of towing vessel William B Klunk drove through the night, declined an offered hotel room, and reported for duty after being awake nearly 14 hours — then took a 15-minute non-operational phone call while navigating a loaded 22-barge tow downbound on the Mississippi. By the time of the collision with moored barges, he'd been awake almost 18 hours. NTSB's report notes that self-assessment of fatigue is inherently unreliable, since fatigue itself degrades the judgment needed to assess it accurately.
Stride — Two Fatalities from an Incomplete Valve Closure
During bunkering aboard the cargo vessel Stride, a valve from repairs six weeks earlier hadn't been fully reinstated, and diesel oil overflowed into the engine room. Investigators believe pressurised hydraulic oil, sprayed from a failed filter box, ignited against the winch engine's exhaust manifold. Three crew were trapped in the engine control room; two died on scene. The vessel's ventilation fans couldn't be shut down remotely, which kept feeding oxygen to the fire.
Cajole — Watertight Integrity Repairs That Didn't Hold
Two weeks before Cajole sank, a Coast Guard inspector found unsealed piping and electrical conduits between the engine room and an auxiliary void, and signed off on repairs the crew had made. Those repairs likely weren't fully effective — water later flooded progressively through the same conduits until the vessel's reserve buoyancy was overcome.
