No One in Charge

On 17 September 2019, two Mid and West Wales Fire and Rescue Service RIBs — N207, a 4.7m Zodiac, and Rescue 1, a 6.4m Delta RIB — were carrying out training and familiarisation manoeuvres on the River Cleddau. There was no overall person in charge of the exercise. The manoeuvres hadn't been briefed to both crews beforehand. Each boat was, in effect, operating to its own plan.

The two RIBs were manoeuvring at speed in close proximity in a confined stretch of river. Neither crew knew what the other intended to do next. They collided. One firefighter was killed; another sustained minor injuries.

A Predictable Consequence, Not a Freak Accident

The investigation's findings were direct: this wasn't an unlucky coincidence, it was the foreseeable result of two independently-operating boats manoeuvring at speed in a confined space with no shared situational picture. Multi-boat training exercises without a designated exercise controller are predictably dangerous — each crew follows its own plan, unaware the other crew has a different one, until the two plans intersect.

The investigation also identified gaps behind the immediate cause: Mid and West Wales FRS's crew qualification requirements for boat helmsmen were inadequate for the risk level of the operations being conducted, and the service's own auditing of boat operations didn't adequately cover these activities. More broadly, there was no nationwide standard for boat operations across UK fire and rescue services at all — meaning the gap that led to this collision wasn't unique to one service.

Recommended Actions

MAIB Report 17/2020 made two recommendations, to Mid and West Wales Fire and Rescue Service and the National Fire Chiefs Council:

  1. 2020/129 — Review crewing and staff qualification requirements for boats, implement measures to maintain crew competency as boat helmsmen, and improve auditing processes to include boat operations
  2. 2020/130 — Adopt a nationwide standard of boat operations for all craft capable of being operated at sea, across every UK fire and rescue service

Human Element Analysis

Communication

Manoeuvres had not been briefed to all participants before the exercise began. Neither crew had any information about what the other intended to do, when, or where — the most basic shared understanding a multi-craft exercise depends on was simply absent.

Situational Awareness

Without a designated exercise controller holding the full picture, no single person on the water had visibility of both boats' positions and intentions at once. Each crew's situational awareness was limited to their own plan — which is not the same as awareness of the exercise as a whole.

Challenge Culture

No qualification framework, auditing process, or nationwide standard existed to prompt anyone — crew, supervisor, or service — to ask whether this exercise needed a controller before it started. The absence of a structure to raise that question meant nobody did.
