The Situation

The pilot is due to disembark in twenty minutes. You go to rig the starboard pilot ladder and find the davit — the mechanism meant to lower and recover it without anyone manhandling its weight — isn't working. The crew have rigged this ladder manually before when the davit's played up. What do you do?

The Lesson

A defective davit is a stop condition, not a workaround — the moment it becomes routine to handle around it is the moment the fall protection it provided quietly disappears.
