The Situation
You're a deck officer aboard a ro-ro freight ferry loading at Gladstone Dock, Liverpool. Your phone rings as you're crossing the stern ramp — the only route to where you're headed — while semi-trailers are actively reversing on and off during loading. There's no physical segregation between the vehicle lanes and pedestrian movement on this ramp.
The Lesson
Physical segregation of pedestrians from vehicle movements in ro-ro cargo operations is essential. Individual vigilance, phone away or not, is not a substitute for structural separation from reversing HGVs.
Human Element
Mobile phone use during cargo operations removed almost all situational awareness at exactly the moment it was needed most, but the deeper failure was structural — there was no physical segregation between pedestrians and reversing vehicles on the ramp at all.
