The Situation

Two months ago, a lessons-learned circular landed on your desk, naming the tanker SEAHAKE: a rescue boat drill at Piraeus anchorage ended with a hidden freewheel defect inside the davit winch, and three crew members injured when the boat's descent went out of control. Your own SMS procedure for boat drills still reads the way it always has: lower to bulwark height, board, then lower to the water. This week's rescue boat drill is due.

The Lesson

A lessons-learned circular only prevents a repeat if it changes the procedure before the next drill, not after one.
